Duration

First line

flucloxacillin PO (IV only if severe)

If flucloxacillin unsuitable / not tolerated:

cefuroxime OR co-amoxiclav PO

5-7 days

+ gentamicin only if signs and symptoms of systemic sepsis

Single dose

+ clindamycin IV if toxic shock

Until shock resolved

Infection near eyes or nose

co-amoxiclav PO

If co-amoxiclav unsuitable / not tolerated:

Azithromycin +/- metronidazole PO

Duration: 5 to 7 days

Full resolution is not expected skin takes time to return to normal

(up to 14 days in total may be needed if systemic signs)

MRSA

clindamycin PO OR co-trimoxazole PO (if sensitive) OR

linezolid PO

vancomycin OR teicoplanin (if septic)

PVL

clindamycin PO

If severe:

clindamycin IV + linezolid IV

Abscess

Boil without cellulitis

Drain – no antibiotics
