If MRSA confirmed or suspected - Add vancomycin IV
Post-operative wound infection - dirty sites

Ceftriaxone IV + metronidazole IV (+/- gentamicin IV if not improving ) for 7 days
If MRSA confirmed or suspected - Add vancomycin IV
