Treatment of first recurrence of CDI

Treatment may not be required if the patient has an equivocal C. difficile result (see Equivocal result). If ongoing symptoms (e.g. diarrhoea), consider repeat testing and seek infection specialist advice as appropriate.

Review severity markers (see Severity of Infection) and contact infection specialist for advice (see How to get advice).

Recurrence of CDI within 12 weeks (= relapse)

Fidaxomicin 200mg every 12 hours orally

Duration = 10 days

Exception – treatment failure identified as incomplete treatment course. Treat as per first episode of CDI with vancomycin 125mg every 6 hours orally for 10 days.

Recurrence of CDI after 12 weeks (= recurrence)

Vancomycin 125 mg every 6 hours orally

Duration = 10 days
