Review Sepsis in Obstetrics

Take blood cultures (10mls in each bottle) in patients with a fever or evidence of sepsis.

Antibiotic recommendation in mild infection

Recommended duration: 7 days

Recommended Antibiotic  Flucloxacillin 1g every 6 hours orally
Penicillin allergy  Clarithromycin 500mg every 12 hours orally

Antibiotic recommendation in severe infection

Recommended duration = IV and oral = 7 days

Recommended Antibiotic  Flucloxacillin 2g every 6 hours IV*
Penicillin allergy  Vancomycin IV* use NHS Lothian AMT intranet calculator choose trough levels 15-20mg/mls.

*Metronidazole can be added if signs of abscess- 400mg Oral, 8 hourly

(500mg IV, 8 hourly if oral route not available). Refer to breast unit if abscess suspected.
