Antibiotic prophylaxis for endocarditis

Warning

The Scottish Dental Clinical Effectiveness Programme (SCEPT) has published national guidance for prophylaxis against infective endocarditis. It has been agreed locally within NHS Lothian/Borders that the following patients should be considered for antibiotic prophylaxis prior to dental and some other invasive procedures

  • prosthetic valve, including transcatheter valves, or where any prosthetic material was used for valve repair
  • previous infective endocarditis
  • congenital heart disease (CHD)
    • any type of cyanotic CHD
    • any type of CHD repaired with a prosthetic material, whether placed surgically or by percutaneous techniques, up to 6 months after the procedure or lifelong if residual shunt or valvular regurgitation remains.

Please refer to SCEDP guideline and contact Cardiology is guidance remains unclear.

Page 13/14 of the SCEDP Implementation Advice document advises on antibiotics regimes. This document can be found under the “Resources and Links” tab.

Who to refer, who not to refer, how to refer

Who to refer:

Identify whether patients meet one of the above categories for antibiotic prophylaxis and refer to SCEDP guidance. Please contact Cardiology if uncertain.
