Diabetes Education for Type 2 Diabetes (Control-IT)

This is an education session for those newly diagnosed with Type 2 Diabetes.

Control-IT is a lifestyle programme for people who have been diagnosed with Type 2 Diabetes. It involves sessions delivered by trained dieticians. The programme aims to give a better understanding of Type 2 Diabetes, and information to help people with Type 2 diabetes manage their condition as well as they can.

Who to refer, who not to refer, how to refer

Who can refer:

We accept referrals from GPs or any Health Care Professional. Patients can also self-refer.

Who to refer:

  • Any patient with a new diagnosis of Type 2 Diabetes (Hba1c >48mmol/mol) within the last 12 months who would like to engage with the Control-IT programme

Who not to refer:

  • Patients who do not have a diagnosis of Type 2 Diabetes
  • Patients who have been diagnosed with Type 2 Diabetes for more than 12 months
  • Patients diagnosed with pre-diabetes, please refer to the weight management service
