Who to Refer:
Adults with a new Diagnosis of Type 2 Diabetes-should be referred to the Control IT programme (via this referral form) -can also be referred via form on SCI-Gateway
Referral criteria for inpatients / outpatients:
- Patients with Type 2 Diabetes on oral hypoglycaemic agents +/- insulin with sub-optimal control
- Patients with newly diagnosed Type 2 diabetes should be referred to Control IT
- Conversions to insulin or GLP-1 therapy (if appropriate)
- Patients with newly diagnosed Type 1 Diabetes
- Anyone with Diabetes on oral hypoglycaemic agents/insulin requiring nutritional support
- Anyone who has diabetes and is pregnant
- Gestational Diabetes (seen in High Risk Pregnancy clinic)
- Those requiring carbohydrate counting advice (1:1 or group sessions)
- Any patient with diabetes and another medical condition e.g. dysphagia, inflammatory bowel disease, renal disease requiring further diabetes input
- Weight management support (may be triaged to Borders Weight Management)
Who not to refer:
For those patients with Pre-diabetes please refer to the Weight management team.
