Who to Refer
- Adults (18+) with symptoms persisting 12 weeks or more following confirmed or suspected COVID-19.
- Patients experiencing multi-system symptoms such as:
- Fatigue
- Breathlessness
- Cognitive difficulties (“brain fog”)
- Sleep disturbance
- Individuals whose symptoms are impacting daily functioning, work, or quality of life.
- Patients who have had basic investigations to rule out other causes. - please see GP GUIDELINE prior to referral to rule out Red Flag symptoms.
Who Not to Refer
- Patients with symptoms lasting less than 12 weeks post-COVID.
- Individuals whose symptoms are fully explained by another diagnosis.
- Patients requiring urgent medical investigation or intervention (e.g. red flag symptoms).
- Children and young people (under 18) – currently not included in the adult pathway.
How to Refer
- Referrals should be made by the patient’s GP, who retains clinical oversight.
- Use the attached referral form.
- Summary of symptoms and duration
- Relevant investigations and results
- Impact on function and daily life
- Any previous interventions or support
- Patients will be contacted for initial assessment and completion of PROMs.
