Falls and general medicine of the elderly

This is a consultant-led clinic for older adults living with frailty (CFS 4 or above) who are experiencing falls or other frailty syndromes, such as:

  • Polypharmacy, multimorbidity and medical complexity
  • Functional decline and immobility
  • Delirium

Who to refer, who not to refer, how to refer

Who to refer:

  • Older adult, living with frailty (CFS 4 or greater) who needs review by a Medicine of the Elderly physician.

Who not to refer:

  • Patients who only need AHP assessment – these should be made to the relevant services
  • If urgent assessment is required in a patient at risk of admission, please discuss with the Frailty Consultant on call via switchboard (bleep 7019)

Primary care management

This is a medical-only clinic, if physiotherapy and/or occupational therapy assessment required please refer for this separately and at the same time as referral to clinic

All referrals undergo Advanced Clinical Referral Triage (ACRT) and you may be asked to supply further information or a referral may be redirected to another service if this is felt to be more appropriate to the patient's needs
