Digital screening tools for patients
This section contains the multimodal prehabilitation screening tools for healthcare professionals to use with patients. All four screening tools should be completed on the first prehabilitation screen.
Referrals for needs-based assessment are not automated through the Right Decision Service (RDS) and it is the responsibility of the local team to generate the referral to the appropriate service.
Key considerations:
- Time point at which prehabilitation screening is completed within pathway to be agreed at local level.
- Screening should be completed as early as possible. This may be at the point of diagnosis or, where agreed locally, prior to a confirmed diagnosis.
- Clinical teams should be aware of the local resources available to deliver prehabilitation at all levels and the referral pathway (including third sector).
- Patients can be re-screened throughout the cancer pathway at the decision of the healthcare team.
- All patients, regardless of screening outcome, should be provided with access to universal prehabilitation including the national cancer Prehabilitation for Scotland website and local universal workshops.
If patients are completing the prehabilitation screening independently via the patient platform, ensure the patient is provided with instructions on how to submit the screening information to the healthcare team e.g. email, print and return.
All forms are also available as fillable PDFs. These can be completed electronically or printed. Scores will need to be manually calculated by healthcare professionals.
