Patient initiated follow-up

All individuals with established Type 1 Diabetes have access to our Annual Screening and Patient Initiated Follow Up System to arrange their own appointments by contacting 0345 602 2812.

We will endeavour to see them within 4 weeks of their call.

This is the information patients are given:

You will continue to be followed up with an Annual Screening appointment and can access our Patient Initiated Follow Up system. This means that should you wish any input from the Diabetes Team in the future you can arrange an appointment by contacting 0345 602 2812. For an NHS NearMe video consultation we will endeavour to see you within 4 weeks of your call. For an in person appointment at Stranraer, Galloway Community Hospital there is likely to be an increased waiting time.

All individuals with established Type 1 Diabetes, and those with Type 2 Diabetes receiving active follow up from the Diabetes Team also have access to our Triage Helpline.

If concerns are more urgent patients can contact the Diabetes Centre on 01387 244214 Monday to Friday 9am – 3.30pm.
