Referral to MDT

Referrals using the order screen can be made to the MDT.

Occupational Therapy, Physiotherapy, Dietitian and Speech Therapy.

Prior to referral ensure relevant risk assessments have been completed.

Example:

Dietician: MUST and Waterlow.

Physiotherapy: Mobility assessment, Falls risk.

Step 1. From ward view on TRAK click on patient, then click on clinical menu.

 

Step 2. In Clinical Menu, click on Order.

 

Step 3. Click on referral to required team.

 

Step 4. Fill in the required information with detailed information regarding why you are referring the patient to that particular team.

Fill out user and password and click on update.
