Post-diagnostic support
Post-diagnostic support is an area of focus for the Focus on Dementia team in Healthcare Improvement Scotland. The team have developed two improvement resources to support people living with dementia and their carers:
- A quality improvement framework for dementia post-diagnostic support in Scotland, and
- A self-assessment tool.
These resources help to improve the delivery of support after a dementia diagnosis.

The purpose of this framework is to set out what is expected from high quality dementia post-diagnostic support.
The framework and self-assessment tool are for practitioners delivering post-diagnostic support and managers of post-diagnostic support services.
