Our transplant unit has its own dedicated Pharmacy team who help look after you and your medicines throughout transplant. To be able to do this safely and effectively, it is important we have a full understanding of your medication history before you come to the ward for transplant.

You should have been given a letter at your 2nd pre-transplant clinic appointment as part of your information pack detailing how we have planned to call you on a Wednesday afternoon, and ask a number of questions to help form your medication history. 

Calling you in advance of your transplant helps us to get the fullest picture of you by looking at the medicines you have been using previously and gathering some information about your medical history and lifestyle.

One of our team will call you between 1pm and 5pm on the Wednesday before you are due to be admitted and ask you about:

  • The medicines you have been prescribed and are currently taking
  • The dose, and how often you take them
  • Any over-the-counter medicines from your local community pharmacy/supermarket (this means things that you buy: such as painkillers, antihistamines, etc.)
  • Any herbal/supplement products

We may also ask you about medicines you have taken in the past and about allergies/sensitivities, as well as previous medical history you may have had (for example, “have you ever been told you have high blood pressure?”).

Please prepare this information using the template provided in the letter.

All information is helpful for us to be able to manage your medicines properly.

We also ask that you bring your medicines to hospital with you on admission, we will usually have a look at them in person and may use them throughout your admission.
