Do I need to admit patients on HEPMA?

No, when patients are admitted on TrakCare they will also be admitted on HEPMA and you will be able to find them through the Inpatient Finder.

If the patient does not show up in Inpatient Finder then they will need to be admitted. Click on Home at the top left of the screen and then click on the Admit Discharge Transfer tile.

How do I record drugs administered during anaesthesia, such as paracetamol IV or an antiemetic?

If you have prescribed a PRN drug, it is not possible to record an administration before the time it was prescribed. You will need to prescribe a Retrospective STAT dose.

  • For example, you may prescribe Cyclizine PRN towards the end of a case but want to record an administration from before the time of prescribing.
  • Retrospective STAT doses can only be recorded for a time that is at least 10 minutes ago.

How do I know if the patient has allergies or sensitivities?

When first opening the patient record a pink Recorded Allergies box will appear if there are any allergies. This will stay until dismissed by clicking on the X in the box.

The Allergy Status field in the patient banner will also show the Allergy state.

Click on the Allergy Status field see more details.

You can add allergies and sensitivities here too. Existing recorded allergies can be amended or deleted if recorded incorrectly.

  • Compound drugs such as co-amoxiclav cannot be recorded as an allergy. You need to record the constituents separately, i.e., clavulanic acid and amoxicillin.

Only one Reaction can be recorded for each allergen.

If more detail is required or the reaction isn’t specified, choose the Other (Add Note) option and create a Patient Note with the extra information.

How do I view or add to Patient Notes?

To view a note, click on Patient Notes from the Inpatient Rx tab.

Notes will be listed on the left. Clicking on a note will show the detail below.

Click on Add Note to create a new note.

Choose a Title, Type of reaction, and enter the details in the box before saving.

Do I have to get the exact preparation correct when prescribing?

Not exactly. If you prescribe 500 mg Paracetamol caplets and there is only 250 mg tablets in stock then they can be given instead.

How do I deal with Drug Conflicts?

When you start the prescription of a drug, you will often be notified of drug conflicts. The effect and severity are displayed.

You will then need to select a rationale for proceeding with the prescription.

Choose a Rationale from the list and tick Apply to ALL to apply to all conflicts.

How do I omit a dose of a drug?

By choosing a start time in the future

You prescribe a Regular drug but also give a STAT dose and then want to miss out one of the regular doses.

For example, you want to prescribe regular Paracetamol at 0800, 1400, 1800 and 2200 but have given a dose IV at 1530 and want to miss out the 1800 dose.

If it is a new prescription, you can change the Start on time to after 1800.

By suspending and then resuming

If the patient is already prescribed and has been administered a drug, you can suspend the drug and then resume it at a specified time.

Right click on the drug and choose Suspend.

Choose a Reason for suspension and make sure the Suspend On time is after the last dose you want given and before the dose you want missed.

The drug will show as Suspended on Inpatient Rx.

Right click and choose Resume.

Choose a Resume On time after the time of the dose you want to omit.

The drug will now show as suspended and there will be an X in the box corresponding to the dose that will be missed.

Once the Resume On time has been reached, the drug will automatically resume.
