Triage failed to attend, Maternity SOP

Objectives

Standard Operating Procedure For when a woman does not attend maternity triage for review.

To outline the process for managing women who have called into maternity triage that are invited in but failed to attend.

Procedure

  • Please consider reason for attendance and any influencing factors for increased vulnerability
  • Consider whether or not transport is an issue – DW hospital co-ordinator re reason for attendance and suitability for potential hospital taxi
  • Within first 4 x hours call the woman back on 1st contact number
  • If call is answered –
    • Ensure the woman is well and give the opportunity to explain why she has not attended. If the woman no longer wishes to attend, discuss importance of initial triage call and advisable to attend.
    • If the woman declines to come in – ensure follow up appointment is in place and reassurance that maternity triage is accessible 24 hours a day, seven days a week. Document clearly in BadgerNet
  • If call is not answered:
    • If no answer leave voice message and attempt a further call back within 6x hours of initial call.
  • If on second call back there is no answer:
    • Check BadgerNet and trackcare to ensure there has been no attendance at another maternity department or another ward area ie A&E/gyn
    • Send text message from generic email address ( if the area has this ) inviting to attend maternity triage due to recent telephone call advise
    • ensure that a follow up appointment is in place with midwife or consultant
    • document clearly in BadgerNet as DNA and attempted two call backs

Roles & Responsibility

Triage midwives have a responsibility to ensure follow up calls and documentation
