My pathway to a healthy weight (in development)

Use this toolkit to help you find your pathway to health and wellbeing and to improving your weight in the way that works best for you. 

These tools and resources are all about helping you to attain the best possible health and wellbeing. 

The toolkit provides:

  • Prompts to help you open up a conversation with your healthcare professional or support team about your health and wellbeing and how you can improve your weight to improve your health. 
  • Tools and resources to help with your psychological readiness for lifestyle change
  • A step by step guide to identify the weight management options in your NHS Board that are suitable for you. This includes links to local and national self-management resources.
  • Signposting to resources about healthy eating and lifestyle options
  • Tools for calculating your Body Mass Index (BMI) and waist to height ratio. These are helpful in determining which healthy weight options are suitable for your needs.
  • A section to help you to define the goals and outcomes that matter to you in achieving a healthy weight and to monitor progress in achieving those goals.
