Thrombolysis actions

• Document initial assessment and prescribe alteplase on thrombolysis assessment document (appendix 1).
• Discuss with patient (if has capacity) and/or relative. If unable to gain consent treatment can be given as an emergency treatment.
• Give alteplase total dose 0.9mg/kg based on actual or body weight. Maximum dose is 90mg. 10% given as bolus over 2 mins followed by remainder over 1 hour started immediately after bolus. (See dosing chart)
• Admit to stroke unit.
• Post thrombolysis observations and care (see nursing protocol on staffnet stroke website).
• Clerk in using stroke clerk in document.
• Request post-thrombolysis CT scan.
