Dundee Dental Hospital Specialist Services: Oral Medicine - Making a Referral

DDH&S accepts referrals for patients residing in Tayside and North East Fife. Patients referred residing out with this area will be considered if the specialist service required is not available in their Health Board and there is agreement with the patient’s local Health Board to fund this treatment along with any patient expenses.

Information Required

Essential information needed for all Oral Medicine referrals:

Patient details:
  • Name
  • Address
  • Date of birth
  • Contact telephone number
GMP details:
  • Name
  • Address
  • Contact telephone number
General clinical information required:
  • A history of the problem precipitating referral
  • Up to date medical history including current medication
  • Use SOCRATES mnemonic as a guide to information required for orofacial pain referrals
  • A history of the problem precipitating referral
  • Indicate clinical features relating to the referral
  • Indicate the provisional diagnosis
  • Indicate treatment carried out to-date and its effect.
TMD referrals:
  • Describe what treatment has been carried out so far e.g. self-management (& splint therapy, if indicated) for 12 weeks unless urgent referral is indicated
  • Indicate the length of time treatment has been carried out and its effect.
  • Indicate the length of time symptoms have been present.
Oral Lesions:
  • Describe the location of the lesion
  • Indicate the length of time the lesion has been present
  • Describe the size, colour, texture and whether it is raised or flat
  • Include a clinical photograph.
