Catchment Area

DDH&S accepts referrals for patients residing in Tayside and North East Fife. Patients referred residing out with this area will be considered if the specialist service required is not available in their Health Board and there is agreement with the patients local Health Board to fund this treatment along with any expenses.

Social & medically relevant information:	Medical problems including allergies Medication taken Smoking history Additional needs (e.g. Interpreter required – please specify which language)
Clinical information required:	A history of the problem precipitating referral Indicate the clinical features relating to the referral and the provisional diagnosis Outline investigations and treatment carried out and its effect
