Warning
Who to refer
- People aged 65 years and over.
- Patients requiring multidisciplinary assessment.
- Conditions such as falls, reduced mobility, multiple medical problems, complications of polypharmacy, frail elderly with uncertain diagnosis, functional decline.
Who not to refer
- People under 65 years, unless they have frailty.
- People who are unlikely to be able to attend a clinic setting, i.e. housebound patients, care home patients.
How to refer
- Referrals are accepted via SCI Gateway, or

Service description
- Assessment Clinic for the Elderly (including the falls clinic) provides a comprehensive geriatric assessment for frail older adults.
- The assessment clinic provides access to specialist multidisciplinary assessment, investigation and treatment.
- Patients attending the clinic are seen by a doctor, a nurse, a pharmacist and a physiotherapist. Patients attending may also be referred onto other professionals are required.
- Following assessment, management plans are completed with each patient.
- A clinic letter will be sent to the patient’s GP.
