Launch of New NHS Scotland National Service

The Percutaneous Mitral Valve and Related Intervention Service

We are pleased to announce that from 30th June 2018, a new Percutaneous Mitral Valve and Related Intervention Service will be launched at the Edinburgh Heart Centre, based at the Royal Infirmary of Edinburgh, with support from NHS Scotland National Services Division. This will provide a comprehensive specialist service for the assessment and percutaneous treatment (minimally invasive) of disorders affecting the mitral valve and related structures for the population of Scotland.

The service covers the following procedures:

  • balloon mitral valvuloplasty (BMV, also known as percutaneous mitral commissurotomy)
  • paravalvular leak closure (PVL closure)

Referral guidelines for cardiologists

We would welcome referrals of patients with mitral stenosis or paravalvular leaks who fit the following criteria:

Mitral Stenosis

Essential Criteria:

  • Symptoms of Heart Failure due to mitral stenosis
  • OR, high risk of embolism
  • OR, symptom limited exercise test
  • AND, clinically significant mitral stenosis (MVA < 1.5 cm2)
  • IF, no unfavourable clinical or anatomical characteristics
  • OR, high risk for valve surgery

Contraindications:

  • Unfavourable clinical or anatomical characteristics (unless high surgical risk)
  • Left atrial thrombus
  • More than mild mitral regurgitation
  • Bi-commissural calcification
  • Absence of commissural fusion
  • Severe concomitant aortic or tricuspid valve disease requiring surgery
  • Concomitant coronary disease requiring surgery

Paravalvular Leak

Essential Criteria:

  • Symptoms of heart failure secondary to PVL
  • OR, Severe mechanical haemolysis requiring transfusion
  • AND, Clinically significant aortic or mitral paravalvular regurgitation (Grade 3/4 or 4/4)

Contraindications:

  • Low surgical risk (re-do valve replacement preferred)
  • Major dehiscence of the prosthetic valve
  • Active infective endocarditis
  • Severe concomitant valve disease requiring surgery
  • Concomitant coronary disease requiring surgery

Patient Pathway

Referrals to the service will be accepted from Consultant Cardiologists and will be expected from all parts of Scotland. All complete referrals to comprise:

  • A detailed written summary of the clinical history including presenting symptoms and details of previous valve interventions and surgeries
  • Relevant blood tests including FBC and electrolytes
  • Recent transthoracic echo
  • A TOE if TTE images are sub-optimal or non-diagnostic
