What do we do?

A community hospital provides care closer to home. Services include:

  • Rehabilitation
  • Recovery after acute hospital stays
  • Medical care
  • Palliative care
  • Post-surgical care

All decision making about your care is shared. Community hospitals promote patient independence and care is individualised. Healthcare staff work closely with local social work teams. Together they facilitate safe discharge processes. Planning for patients' transitions either back home or to alternative care settings.

What will my care look like?

Person-centred care: Your goals and preferences will be discussed and prioritised.

Multidisciplinary team feedback: This will be incorporated into your care plan

Safety: Safety is paramount, with specific interventions, discussions, and risk assessments.

Referrals: Appropriate referrals will be made to address your needs.

Consent and confidentiality: Your consent and confidentiality will be respected.

Communication: Your care will be discussed with you throughout your journey.

Independence: Staff will encourage you to be as independent as possible to maintain your ability to care for yourself after leaving the hospital.

How is my discharge planned?

Planned from admission: Discharge planning starts as soon as you arrive on the ward.

No place like home: Emphasis on returning home or moving to a new setting, supported by carers.

Professional involvement: Relevant professionals will be involved to set your rehabilitation goals and ensure a smooth journey to your discharge destination

Regular discussions: Staff will have ongoing discussions with you and/or your family about your discharge plan.

Flexibility: Plans may change, but any changes will be discussed with you at every stage.

Timely and safe discharge: The team will aim to make your discharge as timely and safe as possible
