The District Nursing Service provides person centred care to people over the age of 18 who are house bound due to frailty, disability or they have palliative care needs best met at home.

Who are the District Nursing Team?

The Team includes Specialist District Nurses, Community Staff Nurses, Assistant Practitioners and Health Care Support Workers.

The District Nurses work closely with GPs, hospitals and other community care services including Social Work.

How and when can I get in touch with a district nurse?

The Teams are based in your local community.

Hours of work are Monday to Friday 8.30am to 6.00pm.

Routine care is undertaken between 8.30am and 4.30pm.

Urgent care is delivered by a smaller team between 4.30pm and 6.00pm.

The out of hours period (weekdays 6.00pm to 8.00am and weekends) is covered by the Evening Service and Out of Hours Nursing Teams who are contactable via NHS 24 on 111.

How long will the District Nurse continue to visit me at home?

We will visit you for as long as nursing care is needed.  This could be until:

  • The health problem we are visiting you for is resolved,
  • You are able to get to your local Health Centre to see a nurse for your treatment,
  • It is more appropriate for you to be cared for by someone else e.g. social care.

Visiting times cannot be guaranteed, routine visiting is between 8.30am and 4.30pm.

Visiting Professionals

Occasionally we may visit with other health care colleagues or students who are learning about community nursing.  Wherever possible we will tell you before this happens.

If you are unhappy about this in any way, then please let your nurse know.

Supplies

You may need equipment, dressings or medicine.  These may be supplied by your nurse or your nurse will advise where you can get them.  This may be your local chemist.  The chemist may be able to deliver these for you if you ask.
