Why am I being discharged so soon?

Hospitals are for acute care. Once you're medically fit, being at home or in a community setting is better for your recovery and wellbeing.

What if I don’t feel ready to go home?

You’ll only be discharged when you're medically fit. If you have concerns, speak to your nurse or discharge coordinator. We’ll ensure support is in place.

Will I get help at home?

Yes. If needed, we’ll arrange care, equipment, or community nursing before you leave hospital.

What is the Planned Date of Discharge (PDD)?

PDD is the estimated date you’ll leave hospital or complete Home First support. It helps everyone plan ahead and ensures a smoother transition.

Can Home First help prevent a hospital admission?

Yes. We can provide short-term support at home to keep you safe and independent, reducing the need for hospital care.

What if I live alone?

We’ll assess your needs and arrange appropriate support. You won’t be discharged without a safe plan in place.

Can my family be involved in planning?

Absolutely. We encourage family and carers to be part of the planning process.

Will I receive visits at specific times?

Due to the nature of the service, we cannot offer timed visits. You’ll be given an approximate timeframe (e.g., morning, lunchtime, afternoon, evening).

What support is not included?

Home First does not provide the following:

  • Overnight visits
  • Domestic tasks such as housework and cleaning
  • Shopping or prescription collection
  • Dog walking
  • Large adaptations to the home
  • Equipment or support for a relative
  • Visits before 8am
  • Visits after 8pm
