You can use this template as a framework and adapt it to fit the scope of the Quality of Care (QoC) review undertaken. If you have a local final report template, you may wish to use this instead. Remember to consider:

• Who is involved with writing and how detailed is the final report?
• Who is it shared with and what is the governance arrangement?
• You may wish to insert the templates you have used during the QoC review.
