Homeless and addictions referral

Patients experiencing homelessness and/or active addiction or substance misuse who are not registered with a general dental practitioner or have specific challenges accessing care via their registered dentist or an alternative local dentist. 

For referrers with access to SCI Gateway e.g. GMPs and GDPs - please refer in through Adult Additional Needs SCI Gateway pathway as outlined in Public Dental Service adult referral criteria.

For referrers who do not have access to SCI Gateway please use the Homeless and addictions referral form.
