Musculoskeletal Services in NHS Ayrshire & Arran - overview

Introduction to clinical pathways

The way in which people living in Ayrshire & Arran with a musculoskeletal complaint can access support is complex and has changed considerably in recent times. There has been considerable growth of the MSK Primary Care services both nationally and locally. In Ayrshire and Arran there is provision for 46 of 53 practices. The national Musculoskeletal Advice and Triage Service, which provided patients with a self-directed access route was stepped down in 2020 and there are no plans for it to be reinstated. Locally, work is underway to develop a safe, effective and efficient process to enable patients to make self –referral to the service thereby reducing unnecessary demand in primary care and out of ours services.

MSK, like many other specialties across our Health and Social Care Service have experienced an increase in complexity of those requiring support. This change in demand to the service has been compounded by the effects of the pandemic, and as a consequence consideration must be given to the changing need of the population.

A review of the clinical pathways has been undertaken to reflect new opportunities to support our population effectively. Advances have been made in digital resources and emerging evidence has been reviewed to ensure care provision is in line with research evidence and best practice guidelines.

Commissioning statements - clinical pathways

  • To provide high value care pathways for the most commonly encountered MSK conditions.
  • To provide patients with high value self-management information.
  • To facilitate and support early management and appropriate work up of patients in primary care, to help support early and appropriate intervention so that treatment is not delayed or patients are directed to
    an inappropriate pathway.
  • Where possible to provide eligibility criteria/referral threshold guidance for referral into secondary care to maximise capacity.
  • To provide expert guidance for MSK clinicians in primary care.
  • To provide expert guidance and information for MSK clinicians in secondary care.

Supporting self management

Many Musculoskeletal problems are self-limiting and will improve with time. To support people to manage their condition effectively we have developed information resources in various formats. We also have representation on the MSK expert panel for NHS24 which continually reviews and improves the information available on NHS Inform. Evidence has shown that people who understand their musculoskeletal health problem and take an active involvement to help themselves have a much better outcome. Supported self-management has a strong evidence base to support the management of MSK conditions at each stage of the patient journey. These
resources are not intended to be a substitute for professional advice, diagnosis or treatment.

Community link workers

The Community Link workers or Community Connector services are an invaluable resource for individuals living in NHS Ayrshire & Arran. They will provide an awareness of what services are available in the third sector locally and support individuals to engage with these. They can support individuals with barriers to
progress in financial, social and welfare needs.

Many options are available to individuals within local communities, online or wider to support meaningful activity.

Better Health Hub

The information and support service of the “Better Health Hub” facilitates health behaviour change and builds health literacy. It is accessible to patients, visitors, family members, carers, volunteers and staff.
It offers:
  • the opportunity to have holistic, person centred discussions about health and wellbeing
  • support to identify what matters to the individual and encourage action to improve health
  • the right information at the right time relevant to individual and their family circumstances
  • referral and signposting to appropriate local services and supports that may be helpful in addressing social determinants of health.
