For urgent social work referrals (not including child protection referrals which are a separate emergency request) please phone the social work team. Weekday in-hours there is a hospital social work team, which can advise which may be the best team to contact. Out of hours there is an emergency social work team that can be contacted through switchboard.

It is best to look at the patient's postcode (or even ask which council collects their bins) to work out which social work team to call.

West Lothian referrals are done through an approved website. Other council referrals are currently organised through TRAK.

NHS Lothian users will no longer be able to select West Lothian Council Services from the Trak Request For Service (RFS) from Tuesday 1st August 2023.

If you raise a request to West Lothian Council, a progress note on Trak must be added. The Progress Note is called: IIE Update, please enter date time and what referral raised for. If this is not completed there is no record on Trak to indicate a referral has been made.
