Bereavement paperwork

The bereavement box now contains the necessary items/paperwork following a patient death in the ED. It is kept in the grab box cupboard. (As of January 2025, paper MCCDs are still in locked cupboard in prep room).

This box now also contains all the information and items related to tissue donation (there is no longer a separate box for this).

I have made a list (attached) of the steps and paperwork to complete following a patient death. There are two copies of this in the bereavement box to refer to.

Please keep the box tidy. Nicky Codona has kindly agreed to check that this box is kept stocked up, so let her know if there is any paperwork missing.

Contacting the GP

On the death of a patient, you should inform the GP.

Within Scotland you should be able to find the GP via their secure NHS Scotland email.
