Discharge against medical advice (DAMA) / self-discharge
Patients wishing to discharge against medical advice (DAMA) should be assessed by, or discussed with, a clinician with experience to ensure that they have a reassuring risk assessment. A DAMA / self-discharge form should be completed and filed with the patients notes.
The form does have guidance on the assessment and management of this group of patients. Whilst not all patients (especially if not waiting for first clinical assessment) will necessarily need to sign a DAMA form, the assessment (and documentation thereof) is important. It is of greater importance in patients with higher risk presentations (e.g. psychiatric conditions, DAMA in conditions where there is high chance of poor outcome). If there is uncertainty please escalate to the most experienced clinician around.
