Diabetes and Endocrinology referrals

Warning

From 20th August 2024, non-emergency referrals to the diabetes and endocrinology teams should be done via TRAK referrals. There will still be a registrar mobile phone in use for emergency/urgent referrals.

How to make referrals

  1. Select patient from floorplan >> Clinical Menu >> Make Referral (note this is different from the standard method of ED ordering)
  2. Choose which service to refer to:
    1. For diabetes advice: In the Order Item lookup >> enter Diabetes Referral >> Select the correct order from the dropdown list - this is likely "Acute Presentation Review"
    2. For endocrinology advice: In the Order Item lookup >> enter and select Endocrine Referral
  3. An Alert may display with guidance on making some referrals - click on this to read it
  4. Confirm understanding of guidance by selecting the tick box >> Update Alerts
  5. select the Questionnaire Icon
  6. Complete mandatory Referral Questionnaire (there is access to the patients EPR on this screen if required)
    1. Click Update when finished
  7. The Questionnaire Icon is now Ticked >> select Update
  8. An alert will pop-up asking "Have you created the Referral Questionnaire?"
    1. If not you can access the questionnaire from the Order Name box
    2. If yes, then select yes, enter your password, and press update
  9. To check the progress of the referral, select patient from floorplan >> EPR >> "Overview/Progress" side tab >> then select the Referrals tab
