Extended guidance on stroke management, over the guidelines - read the standard guidelines first
The guidance on what to give our TIA / stroke patient when they are already on apixaban or other anticoagulants seems a bit vague.
This is a summary provided by our stroke team:

If someone is on apixaban and admitted with a minor stroke/TIA
- keep anticoagulation going but don't add antiplatelets
- If someone hasn't been complying with apixaban (and had a stroke rather than TIA) or they've had a more major stroke

switch to antiplatelets (usually aspirin 300mg) for a period of time before re-anticoagulation given the risk of haemorrhagic transformation.
