Changes to old guidelines

The main changes are that you do not have to scan patients on warfarin/NOAC/clopidogrel with no other symptoms. It should still be considered.

Other considerations

There are a few considerations for patients who do not meet the head injury criteria to get a CT head. These should be discussed with a registrar or consultant. A non-exhaustive list includes:

  1. Patients who require CT scanning for other injuries:
    1. Prudent to include CT head
  2. Patients who you wish to discharge:
    1. if they have good social support, deemed to be able to communicate any new symptoms and seek medical attention then have a conversation with them about the reasons not for scanning and reach a shared decision. Ensure they have written head injury advice
    2. If there are concerns about social support, ability to communicate and seek medical attention then you may consider scanning but shared decision is still appropriate, especially where PoA exists.
  3. Patients who require admission for medical reasons
    1. Do not necessarily need scanning unless other indications for scan develop during their admission
