HPI Vulnerable
Mum has limited understanding of Lewis' language while others have none.
Single clear words (up to 20) but the rest is unintelligible babble. Few two word sentences,
"Papa car", “Papa here”. Little progress noted recently.
He lacks interest of following a two/ three step commands "Go get your shoes and shut the
door". “Can you walk to the door, sit down & clap our hands”.
Can follow simple instructions “Go get your shoes”.
He gets frustrated, squeals & can lash out at mum. HV found Lewis to have limited eye
contact & struggled to listen. Older sibling (who is undergoing assessment for ASD) is asked
by parents to translate what he is saying. He will use gestures or take mum to what he is
trying to communicate when not understood.
Lewis has obsession with having shoes on day & night & in bed, mum is anxious about
Lewis's development and communication.
Mum requesting support.
What are the family, you or your agency currently doing to support this
child?
Speech bundle provided and HV discussed SPEECH tool highlighting specifically the
attention/listening section to then move through all sections of the booklet (Play &
interaction, Receptive, Expressive and Speech sounds).
Nursery application encouraged & peer play through pre 5 groups.
Mum has agreed to work through SPEECH and be review in 12 weeks by HV.

What do you consider another professional can do to help the child's
wellbeing?
Assess Lewis' speech and language development and guide parents on any
advice/strategies/intervention as required.
What difference to the child's well-being is the practitioner (requesting
assistance) hoping to achieve?
In addition to these short-term outcomes, please describe long-term
outcomes:
To reduce Lewis' communication difficulties.
Begin to gain attention, listen and follow instructions.
To increase Lewis' vocabulary so reducing frustration and distress.
To socially communicate with peers through play to establish friendships and to promote a
learning environment for age/stage developmental progress.
