Intro/background

The Public Dental Service (PDS) works in conjunction with General Dental Services and Hospital Services, providing care for those who are unable to access dental care from their local General Dental Practitioner (GDP). The PDS covers a wide range of patient groups and service users with a variety of needs, including domiciliary care.

Patients who are already registered with a local GDP should maintain this registration. The GDP should be the first point of contact for any provision of dental care.

Who to refer

Referral criteria for provision of domiciliary care

This will be based on eligibility and individual dynamic risk assessment by the dental team.

Patients who are unable to access dental care via their local General Dental Practitioner due to:

  • physical disability
  • complex medical conditions
  • acute mental health illness.

In the majority of cases, dental care carried out in a clinical environment offers the safest and most comprehensive treatment options.

Only a limited number of dental procedures are able to be carried out on a domiciliary basis due to access, equipment and safety considerations.

Access to ambulance transport services and specialist equipment, such as wheelchair platforms, within our clinics may allow some patients unable to access their usual dentist to be seen in the dental surgery environment.

  • people in residential care and supported accommodation
  • Any other individuals whose circumstances prevent them from accessing surgery-based care. This may be either long term or short term for example illness or accident.

  • Hospice care team
  • Day centre
  • Dietitian
  • Occupational therapist
  • Residential/care home
  • Speech and language therapist
  • Community nursing team
  • Physiotherapist
  • Any other health care professional.

When & how to refer

For all routine referrals:

It is important that we receive information regarding whether the patient pays for dental treatment or is exempt from payments. (Please refer to the exemption table - Free NHS dental treatment) Details of the next of kin or individual who has power of welfare and/or financial power of attorney rights. The individual/s who has power of welfare and/or power of attorney rights must be fully informed of the proposed treatment and costs by the Care Home prior to the Public Dental Service starting any dental treatment except in case of emergency.

This information will enable us to provide the best possible patient care in a safe and timely manner.

Dental treatment will only be started once all the necessary paperwork has been completed except in the case of emergency dental treatment. If it is deemed by the dentist that it would be in the patient’s best interests to have the dental treatment carried out in a clinic setting then it may be necessary to make arrangements to transport the patient to the most appropriate dental clinic for their dental treatment where possible.

You can get free NHS dental treatment if you:

The proof you need to have

Where and how to get proof

Are under 26 years old

Any official document showing your name and date-of-birth such as a birth certificate, NHS medical card, passport, travel concession card, or any other official papers showing your name and date-of-birth.

NHS medical cards are given out when you are first registered as a patient with a GP. If you need a card, contact your local NHS Board.

Are named on a valid NHS exemption certificate

MatB1 certificate or NHS exemption certificate.

MatB1 certificates are issued by your GP or registered midwife.

You can apply for an NHS exemption certificate as proof. To get one ask your GP, midwife or health visitor for an FW8 application form. The form tells you what to do.

The midwife who delivers your baby will give you a notification of birth form.

Birth certificates and stillbirth certificates are issued by your local registrar of births, marriages and deaths.

Have had a baby in the previous 12 months, or have had a stillborn baby in the previous 12 months

Notification of birth form, birth certificate or stillbirth certificate.

You are named on a valid HC2 certificate

An HC2 certificate for full help with health costs.

Make a claim using form HC1, obtainable from your Jobcentre Plus office or by calling 0131 275 6386.

You may also be able to get an HC1 from your local hospital, dentist, optician or doctor.

 

You are named on a valid HC3 certificate

The charge may be reduced if you are named on an HC3 certificate for partial help with health costs.

You are, or your partner is, getting Income Support

Your Income Support order book, or an entitlement letter from your Jobcentre Plus Office.

You have to claim the benefit at your Jobcentre Plus office (the address will be in your local telephone book).

You are, or your partner is getting income-related Employment and Support Allowance

An entitlement letter from your Job Centre Plus office.

Your Jobcentre Plus office will send you a letter of entitlement.

You are, or your partner is, getting Income-based Jobseeker's Allowance

An entitlement letter from your Jobcentre Plus office.

If payments are made into your bank or building society, you can obtain proof in the form of an entitlement letter from your Jobcentre Plus office.

You are, or your partner is, getting Pension Credit guarantee credit

Pension Credit award notice from the Department of Work and Pensions, Pension Services.

If you have lost or mislaid your Pension Service award notice, contact the Pension Service phone line on 0845 606 0265.

You are, or your partner is, getting tax credits and meeting qualifying conditions

A valid NHS tax credit exemption certificate.

If you are eligible an exemption certificate will be sent to you. See here if you haven't yet received your certificate.

You are entitled to Universal Credit

An entitlement letter from your Job Centre Plus office.

You have to claim the benefit at your Jobcentre Plus office (the address will be in your local telephone book)
